How to spot it
Look for eyes sitting at uneven heights together with sharp teeth showing in an aggressive grin.
Why it matters
This is the kind of bad admit that becomes a much bigger problem later if you ignore a clear face mismatch at intake.
Quick answer
This is a desk-side fail first. If the face, posture, or mouth is already clearly wrong, do not waste extra time trying to let photo or CCTV rescue the call.
Watch a few seconds before deciding
Desk anomalies punish one-glance reads. Hold the patient in view long enough to catch movement, face shape, and obvious extra features.
Trust the desk when the fail is obvious
Do not send a clearly bad face or posture deeper into the route just because you want more proof.

Twitching-style tells disappear if you only memorize still screenshots and never watch the patient idle.

Twitching
Look for fast, erratic twitching in the arms or neck instead of a normal idle pose. The movement can be brief, so watch the patient for four to five seconds before you decide they're clean.
Three Eyes
Some patients show three glowing red eyes instead of the normal two, and it is one of the cleanest desk-check calls in the game.
Hollow Face
Watch for two empty eye holes and a wide-open frowning mouth instead of a normal face.
